Access to rapid and comprehensive tuberculosis (TB) diagnostics remains limited in many low- and middle-income countries (LMICs), particularly in decentralized healthcare settings. While widely used rapid molecular tests can detect TB and rifampicin resistance, many do not simultaneously detect resistance to isoniazid, leaving an important diagnostic gap. Isoniazid-resistant, rifampicin-susceptible TB (Hr-TB) is associated with poorer treatment outcomes and an increased risk of developing further drug resistance.

In response to this need, Stop TB Partnership, in collaboration with Seoul National University and the Respiratory Programmatic Implementation and Research Institute (RPRI) in Indonesia, is evaluating the STANDARD M10 MTB-RIF/INH assay, a rapid molecular test that simultaneously detects TB and resistance to rifampicin and isoniazid. This project supports an implementation study across a hub-and-spoke network of sites in Indonesia to assess the test’s feasibility, its impact on Hr-TB case detection and treatment initiation, and its cost-effectiveness compared with the current standard of care, generating evidence to inform its potential wider use across LMICs.
